The social component of a well-being assessment analyzes the relationship a person shares with the rest of the world. Test how outgoing a person is, how much consideration they give to others and their feelings, and how a person communicates with family, friends, and strangers. Intellectual well-being involves the depths of thought and understanding occurring internally. Explore learning methods, ambition, risk-taking, the formulation of political, social, religious and ethical ideas and beliefs, and an appreciation of the creative arts.

An assessment of environmental well-being refers to the choices an individual makes to preserve the quality of life on earth. An environmentally friendly individual recycles, does not dirty and limits the consumption of energy and water. Spiritual well-being refers to living life with a higher sense of purpose in existence. An assessment of spiritual well-being examines how well a person combines faith with action. Questions about this component of a health assessment seek to ensure that a person enjoys life, recognizes spiritual paths and traditions, experiences an individual spiritual practice, and appreciates the unique beliefs of all people.

A wellness assessment can be done by an individual over the Internet or it can be administered by a company to determine the well-being of employees. This information can be used for studies of corporate well-being as a whole or of well-being in the pipeline within a particular organization. The more information a person fills out about that assessment, the more accurate the results will be. Individuals should complete a wellness assessment honestly and to the best of their ability to allow for lifestyle changes based on the findings.
