Pregnant women may experience back, abdominal, and pelvic pain due to various causes such as hormones, nerve compression, and infections. Pain may indicate serious conditions such as miscarriage, ectopic pregnancy, and preeclampsia, so it’s important to consult a doctor for appropriate management. Gas, bloating, and constipation are common harmless causes of abdominal pain, while preterm labor and placental abruption may cause pelvic pain. Other conditions such as urinary tract infections may also coexist during pregnancy.

A woman may experience occasional pain during pregnancy. The usual types of pain experienced during pregnancy are back, abdominal and pelvic pain. Pain during pregnancy can be due to hormones, nerve entrapment or compression, stretched ligaments, infections, labor, pregnancy abnormalities, and illnesses. It can also be associated with other symptoms, such as bleeding, spotting, or fever. When a woman experiences pain during pregnancy, it is important that she consults with a doctor to identify the specific cause of the pain and receive appropriate management.

Back pain occurs in about half of pregnant women. It is usually due to weight gain, the hormone relaxin, or nerve compression. The characteristic of pain depends on its location. Upper back pain is usually described as burning, while lower back pain is usually described as dull. Buttock pain, usually described as stabbing, may also exist.

Abdominal pain during pregnancy can be due to a harmless condition or a serious one. The most common causes of harmless abdominal pain during pregnancy are gas and bloating, constipation, and round ligament pain. Gas, bloating and constipation are due to the enlarged uterus putting pressure on the stomach and intestines. Round ligament pain is due to stretching of the round ligaments due to the enlargement of the uterus.

Serious conditions that can cause abdominal pain during pregnancy include miscarriage, ectopic pregnancy, and preeclampsia. Abdominal pain in miscarriage is characterized as cramping and widespread and is often associated with vaginal bleeding. Ectopic pregnancy is a condition in which the implantation of a fertilized egg takes place outside the uterus. Preeclampsia is an abnormal condition in which a pregnant woman has high blood pressure and protein in her urine. It can also present with headaches, nausea and vomiting, vision changes, and swelling around the eyes, hands, or feet.

Pelvic pain during pregnancy is usually due to preterm labor and placental abruption. Miscarriage, ectopic pregnancy, and round ligament pain can also present with pelvic pain. In preterm labor, pain is characterized by menstrual cramps. Placental abruption is an abnormal condition in pregnancy in which there is partial or complete separation of the placenta from the uterus before delivery.

Other conditions that present with pain may coexist during pregnancy. Some of these conditions are rupture or torsion of the ovary, viral gastroenteritis, irritable bowel syndrome, appendicitis, and urinary tract infection. Urinary tract infection is common among pregnant women due to the compression of the uterus enlarging on the bladder. It usually presents with foul-smelling or bloody urine, pelvic pain, urinary frequency and urgency, and burning pain when urinating.
